I know something like 'attached herewith are my X documents' is not uncommon in Filipino English If you use it that way, you need a verb ( is are ) and you need something that is a file or document ('Registry of Workers, Assessed and Certified' isn't clearly one - perhaps you mean 'Register of Workers' or RWAC document?)
